Upon receipt of this registration form, PI-21 will email an invoice for the course to the email address provided.  Payment of the invoice will be required prior to the start of the course.  We accept check, Visa, Master Card, Discover, and American Express.  A secure online payment option is provided in the email with the invoice, use the link provided to pay by credit card.

Refund Policy:

To receive a refund you must notify us in writing of the cancellation at least three weeks prior to the start date. All refunds are subject to an administration fee of $50.00. No refunds will be issued after the three week grace period. Attendee may, however, choose to transfer to a later date (to be scheduled at a later date) or may send a substitute. All attendees must send an E-mail notification to informing us of your intent to cancel, transfer, or send a substitute.
